Nowadays, Penoplasty has gained momentum as a solution for improving penile length and girth. Improvement in techniques played an essential role in patient safety with a satisfactory result.

What is Penoplasty?

Penoplasty is a male intimate surgical procedure that enhances and improves the aesthetic appearance of the genitals through increased length and girth. Moreover, it is carried out under local anaesthesia with moderate sedation. 

What is moderate sedation?

Moderate sedation is “a drug-induced depression of consciousness during which patients respond purposefully to verbal commands, either alone or accompanied by light tactile stimulation” (Moderate and Deep Sedation in Clinical Practice; Urman et al.; 2012; p. 4). In short, patients are in a deep sleep and feel no pain, but can still respond to verbal commands, touch or both.

During sedation, spontaneous breathing is not affected (unlike general anaesthesia where interventions are often required), and supplementary oxygen is administered through a nasal cannulae only. 

How is Penoplasty done?

Penoplasty involves two procedures: lengthening and enlargement.

The lengthening is done through the release of the suspensory ligament which fastens the penis to the symphysis pubis. Once the ligament is released, there is an increase in length externally by one to one and a half inches. It is important to note that the penile length gained after Penoplasty is the same when the penis is flaccid or erect.

On the other hand, the enlargement is done through fat graft, which is obtained by liposuction from other areas such as the abdomen, flanks, or thighs. Once the pure fat has been collected, it is then filtered and may be combined with stem cells or platelet-rich plasma, which enhances the viability of the fat. Lastly, the fat is injected on the shaft. 

How long is the procedure?

Depending on whether you are doing enlargement, lengthening or both, the procedure can take from 45 minutes to two and a half hours.

How is recovery?

A key factor for sedation is its ability for a fast recovery. You will need to recover for a few hours at the clinic before you can go home. We also advise you to have someone drive you home and stay with you for the first couple of days. 

A week off from work is recommended for recuperation and rest. For enlargement, it takes three to four weeks; for lengthening, five to six weeks to resume an intimate relationship. 

Is Penoplasty permanent?

The length gained after Penoplasty is permanent. However, penile enlargement involves subsequent reinjection of fat, one month apart until the patient has achieved the size they want. The number of reinjections can vary from person to person. Generally, after the first injection, there is around 30% fat absorption rate. Due to this, the surgeon will inject the maximum amount of fat on the first treatment.

When will I see the result?

It will take a few weeks and sometimes even months, for the new penis to take its final size and shape. 

What are the risks and complications involved?

Bruising and swelling are the most common. These will subside approximately in a couple of weeks. 

Pain and discomfort, which peak at approximately the second to the third day, are also expected and can be managed through oral pain medication. 

You may also experience lumps and bumps around the shaft. Massaging the shaft with Vaseline for a few minutes every day can help resolve this issue.

Although rare, infections and accumulation of fluid (seroma) can happen. Infections are dealt with using antibiotics, while seroma is managed through fluid aspiration.
